CHEYYAR HUD - GUIDELINES FOR ISSUING FREE BIRTH CERTIFICATES

As per the letter R.No. 47721/SBHI-I/S1/2009, Dated: 18.05.2009 of the Director of Public Health and Preventive Medicine, Chennai-6. the following Guidelines may be observed for issuing Free Birth Certificates.

1.Every year April/May months are to observed as Free Birth Certificate issuing months.
2.IEC Activities to be carried out on the importance of registering all Births and Deaths in time.
3.To ensure that the Birth & Death certificates are issued immediately after the registration.
4.To inform the parents about the name inclusion of a child within a year at free of cost.
5.To take necessary action for registering the unregistered events.
6.The importance of above activities shall be made known to the public through “Tom Tom”, Hand Bills, Rallies and local TV channels.
7.The same may also be discussed at the “Grama Sabha Meeting”.
Steps to be taken by All Tahsildars, Municipal Commissioners and Executive Officers of Town Panchayats:
i.All Tahsildars are requested to convene a meeting for all VAOs, RIs in their jurisdiction and to inform about the issue of Free Birth Certificates to all children upto 5 years immediately after the registration.

ii.All the Municipal Commissioners and Executive Officers of all Town Panchayats are requested to issue Free Birth Certificates to all children upto 5 years immediately after the registration.

iii. As a special case if the Birth register is not available in the concerned office from the year 2003 action may be initiated by all Tahsildars and Executive Officers of Town Panchayats to get it from the concerned Sub Registrar and hand over it to VAOs /Sanitary Inspectors for inclusion of name and issuing Free Birth Certificated upto 5 years.

iv.All Tahsildars should get required Birth Certificates forms from the office of the District Revenue Officer and distribute to all VAOs.

v.All Registrars should maintain a register about the issue of Free Birth Certificates.

vi.Action may be initiated to issue Free Birth Certificates within a week for the children born during April/May of this year.

vii.The Chief Educational Officer, District Educational Officer and all Elementary Educational Officers are requested to insist all school authorities to ask for Birth Certificates at the time of school admission without fail.

viii.A report may be submitted to the Deputy Director of Health Services, Cheyyar about the No. of Certificates issued.
